35歲前，黃金生育期
國內生育率低，醫師也呼籲，有心當媽媽的婦女不要蹉跎「生」機，35歲前是生育的黃金年齡，等到40歲以後，受孕機率像溜滑梯一路走下坡；即使受孕，流產機率也會提高，新生兒死產率甚至可能高達四成。
婦產科醫學會理事長蔡鴻德表示，當高齡產婦不僅挑戰體力，也要看運氣。女性從青春期開始排卵，卵子已經在「等待」，35歲以前其實是最佳的黃金生育年齡；隨年紀漸長，卵子品質也在下降中，高齡產婦可能提高胎兒罹患唐氏症的機率。
開業婦產科醫師李茂盛也說，門診中有婦女一直猶豫，「10年前我就勸她生」，等下決心已經來不及。他強調，女性34 歲以後由於卵巢功能退化，流產機率比一般正常孕婦多了2 倍。
若是等到40歲以後再懷孕，新生兒的死產率甚至可能提高至40%。除了女性先天條件越來越不利懷孕，男性的精子在40歲以後，活動量也會減少，男女雙方的生育條件，都不利有「生」機。

想生再生？小心卵巢提前停工
現代女性常晚婚，對生育更抱持「隨緣」態度，總覺得「想生再生」，不過小心卵巢可能提前「停工」。
婦產科醫師最近常替35歲以上婦女抽血檢測荷爾蒙，得知卵子「庫存量」，如果卵子數量稀少，就算比一般女性多打四倍劑量的排卵針還是沒用，只能靠借卵才可能一圓當媽的心願。
不少婦女覺得，想生的時候再努力嘗試，不然就花錢做試管嬰兒，對當高齡產婦一點都不在意。開業婦產科醫師李茂盛表示，門診中就曾遇過46歲的婦女，本來沒生小孩打算，但看朋友的兒女漸漸長大，想法也改變，才到門診積極求子。
不過即使還沒停經，想仰賴生殖科技，也要看卵巢功能配不配合。李茂盛說，婦產科門診近來常會建議有意生育的婦女，可以先抽血檢測「抗穆勒式荷爾蒙」(AMH)，預測卵巢老化程度。AMH如果降到1以下，通常就沒有機會。
根據門診經驗，幾乎43歲以後，AMH都降到「警戒值」。這時除非借卵，幾乎已沒有「子」望。
李茂盛說，卵巢裡有許多原始卵泡，估計青春期大概有40萬個，但「一將功成萬骨枯」，眾多原始卵泡中，僅有少數會成為卵子，而且卵泡會隨年齡消耗，大約50歲左右就沒了，開始進入更年期。當AMH檢測值高，代表卵巢內原始卵泡數量多，還有機會排卵，AMH低就是卵子「庫存量」低的警訊。
婦產科醫學會理事長蔡鴻德也說，AMH本來是治療不孕症的常規檢查，預測卵巢功能準確度高。但由於國內婦女普遍晚婚，因此門診中醫師也會用來評估一般女性的生殖能力，提醒卵巢可能已經提早「收工」，想生就快。
他的門診中也有患者，因為AMH太低，即使早上打四支、下午打四支，比一般人多打高達4倍劑量的排卵針，婦女苦於腹痛、噁心的嚴重不適，還是無法刺激排卵。
